It is the official record of how to collect and report on the indicators that have been selected, and ensure continuity and institutional memory.

Use this as a sample to develop a national or programme-specific Indicator logical framework (logframe). This lists each indicator with its baseline information and targets for the duration of the project or programme. It is suggested as part of any M&E plan.

M&E Work Plan & Budget (Road Map)

Use this sample to develop a costed or budgeted M&E work plan. It is suggested to include an M&E work plan (or an abbreviated version of it) within the national HIV/AIDS M&E Plan or the national strategic plan for HIV/AIDS, and serves as a guide for all stakeholders to use in developing their individual M&E work plans or in budgeting for M&E activities.

Program monitoring data collection tool, guidelines and reporting format

Use this outline to develop tools, guidelines and reporting formats for programme monitoring. Indicators can be adjusted to cater for specific donor-funded projects (i.e., GFATM, PEPFAR, etc) or for overall national programme monitoring, and can be used for HIV/AIDS, TB or malaria programmes. Be sure to add all necessary indicators on the data collection forms, and full descriptions of each indicator on the guidelines. Only one form can be used for an entire sector or disease programme; thus, all organisations reporting to the national programme could use this HIV/AIDS form to report, completing only those sections applicable.

Consulting report format

This format can be used to write trip reports or other end-of-project summaries that do not offer prescribed report formats. It is good practice to offer a brief summary of the trip/work completed whether to satisfy donor requirements or simply to brief the client on trips or activities.
